Routine maintenance. I've seen many organizations that will create a system and neglect all routine maintenance of the equipment. You can have the best system, but if batteries and the devices are not checked on a routine basis, the system will fail. I have a client with a 490-million-dollar system but has problems with communications. No one checks the devices or the batteries, which, once I got called in and checking found out the batteries were over 5 years old and many of the user devices had issues.
